Role Overview:
This position involves working within a small, collaborative team, managing a focused caseload of 6-8 cases. You will carry out thorough assessments, develop tailored care plans, and work closely with health, education, and external agencies to deliver impactful support.
The role offers a hybrid working arrangement, with four days from home and one day in the office. Office days (Tuesdays) are dedicated to team supervision, peer support, and professional development activities.
