💡 About the Role
You’ll be joining a proactive and supportive Access and Wellbeing Team focused on prevention, enablement, and strength-based support. This includes medium to longer-term social work interventions, safeguarding enquiries, reassessments, and reviews.
📌 Key Requirements
🔹 Must be available to be physically present in the office 3 days per week
🔹 Role includes:
- Home visits 🏠
- Joint working with other professionals 🤝
- Duty responsibilities 📞
✅ About You:
- Qualified and progressed Social Worker, registered with Social Work England.
- Completing Care Act assessments 📋
- Court of Protection and Human Rights assessments ⚖️
- Undertaking Carers Assessments
